There are
many types of documents with varying types of documentation that hospitals and
medical establishments keep, both, for the benefit of patients and for the
benefit of the establishment. One of the
most obvious locations within which documentation is located is a patient’s
medical record. And, it is within a
patient’s medical record where a reason for a fee reduction would be located.
